CONSENT TO THE PROCESSING OF PERSONAL DATA INCLUDING THE INFORMATION OBLIGATION (RESULTING FROM THE GDPR) 

The administrator, i.e. the entity deciding for what purposes and how your personal data are processed, is Derek Gurry, ul. Tarasowa 8, 83-314 Slawki

AND.

1. Derek Gurry processes your personal data for the purposes necessary to carry out the agreed forms of meetings (consultations, individual, group, couples sessions, other forms of meetings, including online).
2.  necessary for the performance of contracts, b) fulfillment of the legal obligations incumbent on the controller (legally required registers of commercial transactions, invoices, tax settlements, etc.), c) marketing of the controller’s products and services – providing customers with information and offers regarding our products (legitimate interest of the data controller), while respecting other legal provisions regulating these activities.
   
   
3. The recipients of personal data will be persons authorised by Derek Gurry to process personal data as part of their official duties (employees and associates), as well as third parties to whom Derek Gurry commissions the performance of activities involving the processing of personal data (including entities providing or administering IT systems used by Derek Gurry as part of its business).
4. Processing of your personal data:
   • results from your consent to the processing of your personal data for the purpose indicated in paragraph 1 above;
   • it is necessary for the performance of a contract to which you are a party or to take action at your request before concluding a contract;
   • constitutes the implementation of the legitimate interests of Derek Gurry, Gdynia as the controller of your personal data.

Legal basis for processing: Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (“GDPR”).

Derek Gurry processes the following categories of your personal data:

1. Basic identification data: name and/or surname, job title, date of birth.
2. Contact details: e-mail address, telephone number, home or mailing address.
3. Other data provided to Derek Gurry shall have the right to delete the data referred to in the preceding sentence and which, pursuant to the decision of Derek Gurry, will not be further processed.

Your personal data will be processed for the period necessary to achieve the purposes indicated in point II above and for a period no longer than 5 years from the end of contact, or until Derek Gurry receives your objection to further processing. 

We process your personal data only to the extent of:

Principle of minimization, data storage period

When processing your personal data, we are guided by the principle of minimization, which means we collect it only when necessary, to the minimum extent necessary to achieve the purpose and for the minimum storage period.

Derek Gurry ensures that all persons whose personal data is processed have the appropriate rights under the GDPR, i.e.:

1. the right to access personal data;
2. the right to request rectification (correction) of personal data – if the data is incorrect or incomplete;
3. The right to object to data processing. You have the right to object to the processing of your data. To do so, contact Derek Gurry at +48 733888339 or by email: info@GrowWithGurry.com   . We will then cease processing your data for the previous purposes, unless we demonstrate that the basis for our processing of your data is necessary to establish, pursue, or defend Derek Gurry’s claims.               
4. the right to request the deletion of personal data;
5. the right to request restriction of the processing of personal data. At any time, you may request that we limit the processing of your personal data solely to storing it or performing other agreed-upon activities;
6. At any time, you may request access to, rectification of, deletion of, or restriction of processing of your personal data by sending a request to rodo@oddk.pl . You may also file a complaint with the supervisory authority – the President of the Personal Data Protection Office.
